Alumni Transcript Request

Former students and graduates of Boiling Springs High School may request an official transcript through the Counseling Office. Please review the instructions below before submitting your request.


How to Request an Official Transcript 
Option 1: Submit the your request through our Google Form Alumni Transcript Request Form 

Option 2: Download the PDF and complete the Transcript Request Form (attached at the bottom of this page)  

1. Submit a Transcript Request

Option 1 – Google Form

Complete the Online Transcript Request Google Form Alumni Transcript Request Form
All required information must be entered for processing.

Option 2 – PDF Form
Download the PDF or pick up the Official Transcript Request Form from the High School Office

Complete all required fields, including: 
Provide the following information:

  • Full name at the time of attendance
  • Date of birth
  • Date you are submitting the request form
  • Year of graduation
  • Current contact information

Submission options for the PDF form:

  • Email the completed form to [email protected]
  • Drop the completed form off at the High School Office
  • or Mail the completed form to Boiling Springs High School Attention: Counseling Office 21 Academy Street, Boiling Springs, PA 17007
2. Provide Recipient Information 
    Clearly indicate where the transcript should be sent.
    Include the full mailing address or secure email address.
    3. Authorization Requirement 
    A signed request is required by law before transcripts can be released.
    Requests without a signature will not be processed. 
    4. Processing Time 
    Please allow up to 10 business days for processing.
    Extra time may be required during busy periods (college application season, beginning/end of school year, summer, etc.).
    Transcript requests must be submitted at least 10 business days prior to any deadlines 
       
    5. Delivery Options 
    Transcripts can be:
    • Mailed directly to the requested institution
    • Sent electronically directly to the requested institution
    Reminder: Transcripts are considered official only if sent directly by the high school or provided in a sealed, unopened envelope.
