Opt Out Form

We suggest students select books that are of interest to them and meet their family’s beliefs and/or values. We greatly appreciate the efforts of parents/guardians who speak to their students regarding the type of material you are comfortable with them reading.
If you want your student’s access within the BSHS school library (print and digital) to remain the same, then you DO NOT need to do anything further.

If you would prefer that your student not be permitted to check out Mature Young Adult and Adult Literature, we will put a private note on their account to prevent checkout of those materials.

If you DO want your student to have content restrictions, the following options may be selected on the Mature Content Opt Out Form:

Option 1: If you feel your student should NOT be allowed to check out Mature Young Adult and Adult Literature, please fill out the BSHS Library Mature Content Opt Out Form below. Your student will be identified privately in our circulation system which will restrict their access to Mature Young Adult and Adult Literature.		Option 2: Digital content (eBooks and audiobooks) cannot be categorized in the same way as print books. If you feel your student should NOT be permitted to check out digital content (eBooks and audiobooks) because we cannot categorize them, please fill out the BSHS Library Mature Content Opt Out Form below. Your student will be blocked from BSHS titles in MackinVIA which will restrict their access to ALL MackinVIA eBooks and audiobooks, not just mature titles.

Note: This form needs to be completed each school year for each BSHS student and should be returned to Mrs. Bond in the BSHS Library. Mrs. Bond will send an email confirming receipt of any submitted form(s) - typically within 24 hours - to the parent/guardian who completes them.
